Wisconsin Department of Justice, Office of Crime Victim Services, Application for Crime Victims’ Compensation is the form used to apply for reimbursement for medical & mental health expenses, lost wages and benefits and funeral expenses for adults and children who are victims of crime and have no other sources of payment for these services or losses.
